As a Care Manager, you are responsible for the day to day operations relating to client care delivery and caregiver management. Reporting to the Administrator, the Care Manager provides leadership, support, and consultation to clients and caregivers. You will drive growth through the following main duties and responsibilities: - Conducts client care assessments with respect to physical, emotional, and environmental needs and advises on client care - Based on the care assessment, develops a comprehensive care plan, schedules staff accordingly, and attends first visits to introduce caregivers to clients - Coordinate staff training, and development, supervise staff to ensure their care is aligned with best practices in accordance with the profession - Preform administrative functions including payroll, scheduling, and billing. - Maintain electronic health records for clients and employment records for caregivers - Recruitment functions including hiring, interviewing, and preforming orientations for new staff **Qualifications**: - College diploma or University Degree in a related field I.E. RPN, Social Work, PSW, DSW, etc. - Experience working in a healthcare environment - Experience working with seniors and aging populations is preferred - Possess and maintain a valid Class "G" Ontario Drivers License, or equivalent and access to a vehicle (paid mileage) - Strong computer skills, proficient in G-Suite: Gmail, drive, docs, calendar. - Exceptional written and verbal communication skills **Availability**: - Hours of work are 9:00am - 5:00pm Monday to Friday with occasional weekend/evening coverage **Condition of Employment**: - This position shall be conditional on a satisfactory Police Vulnerable Sector Check (PVSC) to ensure the absence of relevant criminal convictions. (within the last twelve months) - Negative TB test (within the last twelve months) - This position shall be conditional upon proof of COVID-19 vaccination or valid medication exemption. **Salary**: From $30.00 per hour **Benefits**: - Dental care - Extended health care - On-site parking Schedule: - 8 hour shift Supplemental pay types: - Overtime pay COVID-19 considerations: All staff wear PPE including gloves and masks when working with clients. All staff are immunized for COVID-19.
