Position: Case Manager Location: Muslim Welfare Residences, 730-740 Dundas Street East, Whitby, Ontario, L1N 2J7 Salary: $47,250 (CAD) per annum Application Deadline: January 15, 2025

**Job Description**:
Daily Responsibilities:

- Conduct morning meetings with the Senior Administrator.
- Answer phone calls and manage walk-in clients.
- Update the room map and write the communication log with the Housing Coordinator and Mental Health Counsellor.
- Hold brief afternoon debriefings for critical issues.
- Manage client concerns and complaints during case working time, and list them for weekly reports.
- Educate clients about Ontario Housing and other agencies.
- Share clients’ meeting schedules with the Senior Administrator.
- Provide daily updates to the Senior Administrator on casework progress.
- Update filings, formats, questionnaires, and paperwork as needed.
- CC the Senior Administrator on all correspondences for record-keeping.

Weekly Responsibilities:

- Conduct unit visits to engage with clients not scheduled for meetings, recording their needs.
- Complete 2 to 3 case management meetings per day, encouraging clients to advocate for themselves.

General Responsibilities:

- Set clients’ goals and work with them to achieve these goals.
- Update HIFIS accordingly (training will be provided if required).
- Educate clients on community behavior, hygiene, health maintenance, and property upkeep.
- Identify serious issues and refer to the in-house counsellor.
- Refer clients’ needs to the Senior Administrator and community resources.
- Discourage bringing donations and food from outside sources.
- Guide clients on whom to approach for various needs within the residence.
- Regularly follow up on clients’ improvements.
- Prepare consent papers for clients’ safety.
- Issue warnings and strikes to clients as necessary.
- Write announcements and design flyers for events and activities.
- Conduct in-unit visits to ensure program requirements are met.
- Prepare and share weekly case management reports with the Senior Administrator.
- Prepare monthly progress and staff assessment reports.
- Perform other official duties in coordination with staff

Required Qualifications:

- Bachelor’s degree in Social Work, Psychology, or a related field.
- Minimum of 2 years of experience in case management or a similar role.
- Strong understanding of Ontario Housing and community resources.
- Excellent communication and interpersonal skills.
- Ability to manage multiple tasks and prioritize effectively.
- Proficiency in Microsoft Office Suite.
- Experience with HIFIS is an asset (training will be provided if required).
- Ability to work independently and as part of a team.
- Strong organizational and problem-solving skills.
- Compassionate and empathetic approach to client care.
- Canadian citizenship or permanent resident status.
- Valid Ontario driver’s license.
