Quality Improvement Lead Job Description

Quality, Risk and Innovation

As the quality improvement lead you are tasked with overseeing and driving the quality improvement measures within the home. You will accomplish this by assessing the environment to identify opportunities for improvement to enhance resident care and experience outcomes. You will act as the catalyst for quality and innovation processes within the home by streamlining and aligning all disciplines to commit to a united goal / outcome. You will use project management frameworks and tools to assess, understand, communicate, plan, execute and evaluate quality improvement measures within the home.

RESPONSIBILITIES
- Oversee quality initiatives across the home with focus on Balanced Score Card (BSC) indicators
- Access and implement project charters developed by others in the region
- Lead the evolvement and refinement of an assigned project charter
- Check-in with other homes from the region on their implementation efforts
- Track and trend quality indicator performance monthly
- Update quality improvement spreadsheet with process and progress monthly
- Participate in monthly regional quality improvement calls
- Collaboration with other QI leads in region to share successes whether remotely or in person
- Must be able to work flexible hours to meet the needs of the homes to work on QI initiatives or studies
- Attend and participate in preexisting committee meetings associate with BSC indicators
- Assemble a core QI committee and meet monthly (focus on frontline staff)
- Perform environmental scans to identify root cause problems and deficiencies that impact quality by,
- Be present on the units
- Converse with frontline team members
- Observe care and other practices (active observation vs passive observation)
- Use various modalities to define and understand problems (5 whys, fishbone, spaghetti diagram, etc.)
- Be a coach and a mentor
- Develop trust with all stakeholders
- Identify gaps in training and education that impact quality
- Report findings to leadership and be part of the solution
- Must have strong organizational skills and attention to detail
- Be able to communicate effectively and timely
