**Position Summary**
**Key Responsibilities**
- **Resident Care Leadership** - Oversee all aspects of resident care, ensuring safety, health, and overall well-being.
- **Team Coordination & Support** - Guide and collaborate with care staff, ensuring training, mentorship, and adherence to care standards alongside the Manager of Operations.
- **Family & Healthcare Liaison** - Act as the primary point of contact for Extramural, pharmacy, physicians, and families regarding resident health and care needs.
- **Admissions Oversight** - Coordinate the admission process in partnership with the Manager of Operations, ensuring smooth transitions for new residents.
- **Policy & Compliance** - Ensure policies, procedures, and licensing standards are consistently followed.
- **Documentation & Care Plans** - Develop and maintain accurate and timely records, including care plans, assessments, and progress notes.
- **Quality Assurance** - Regularly monitor care practices, identify areas for improvement, and recommend enhancements to ensure excellence in service delivery.
- **Transportation Coordination** - Manage and schedule resident transportation to medical appointments using the company vehicle.
- **On-Call Rotation** - Participate in the on-call leadership rotation approximately one (1) week in every three (3), providing after-hours support and guidance as required.
**Qualifications**
- **Education**:Applicants must be a Licensed Practical Nurse (LPN) or hold a higher healthcare qualification, with an active license in good standing.
- **Experience**: 2-3 years previous experience in a similar role within healthcare, preferably in a Special Care Home setting.
- **Skills**: Strong leadership, organizational, and communication skills.
- **Knowledge**: Understanding of care standards, policies, procedures, and risk management in a residential care environment.
- **Attributes**: Compassionate, empathetic, detail-oriented, and able to work both independently and as part of a team.
- **Other Requirements**: Valid Class 5 driver’s license with a clear driver’s abstract.
