**Reports to**:Gateway House Manager

**Responsibilities**:
The Youth Service Coordinator is responsible for providing coordinated, time-limited, and outcomes-focused support to youth accessing a short-term transitional shelter. This role ensures a consistent and standardized intake process, timely assessment of immediate needs, and rapid connection to appropriate community supports in alignment with the MSS service delivery expectations.

The Youth Service Coordinator provides structured case management, service coordination, and individualized support to assist youth to stabilize, address immediate barriers, and transition successfully to longer-term housing and community-based services.

**Qualifications**:

- Post-secondary education and/or relevant experience;
- Experience in working with at risk youth;
- Open minded and willing to learn;
- Proficient computer skills;
- Knowledge of community resources, supports and other agencies;
- Excellent time management skills;
- Ability to work independently and as part of a team;
- Strong assessment, documentation, and organizational skills;
- Ability to work effectively in a fast-paced, short-term, shelter-based environment;
- Excellent communication, written and interpersonal skills are essential;
- Valid driver’s license;
- Must be able to work flexible hours when needed; and pass YWCA Security;
- Clearance with a successful vulnerable sector and criminal background check;

**Duties**:
**Intake, Assessment, and Case Planning**
- Complete standardized intakes in accordance with MSS requirements and organizational policy;
- Conduct timely, comprehensive assessments to identify immediate needs related to housing, safety, income, health, mental health, addictions, education, employment, and cultural supports ;
- Develop individualized, short-term support and transition plans aligned with the program’s limited length of stay ;
- Establish clear, measurable, and realistic goals focused on stabilization and transition readiness ;
- Ensure accuracy, consistency, and timeliness of intake documentation, assessments, and case planning tools;
**Individualized Support and Skill Development**
- Provide time-limited, one-on-one support focused on stabilization and preparation for transition;
- Support youth to develop practical life skills, including budgeting, basic financial management, job readiness, time management, and daily living skills;
- Tailor supports to each youth’s immediate priorities, strengths, and identified needs;
**Service Coordination and Transitions**
- Identify and facilitate timely connections to appropriate community-based services to prevent service gaps;
- Coordinate communication between youth, internal staff, and external service providers;
- Support youth to exit the program with clear next steps, referrals in place, and aftercare connections where applicable;
**Crisis Response and Safety**
- Support youth during periods of crisis using trauma-informed and de-escalation approaches;
- Assist with safety planning and provide follow-up after incidents to reduce repeat concerns;
- Collaborate with program staff to ensure consistent, appropriate responses to safety and behavioral issues;
**Programming and Engagement**
- Coordinate and support in-house workshops, group activities, and engagement strategies appropriate to a short-term shelter environment and identified youth needs;
**Documentation, Compliance, and Reporting**
- Maintain accurate, timely, and detailed case notes in accordance with MSS standards and privacy legislation;
- Track outcomes related to goals, service connections, and transitions;
- Contribute to reports, audits, and data collection as required by MSS and the organization;
- Ensure compliance with organizational policies, funder expectations, and legislative requirements

**Team Collaboration and Professional Practice**
- Work collaboratively with Outreach Workers, Program Staff, and management;
- Participate in case conferences, team meetings, and program planning;
- Maintain professional boundaries and ethical standards

**Probation**:All YWCA Prince Albert positions contain a three-month probationary period.
