Oakville, Canada Latitude Child & Youth Services Full time

Latitude Child and Youth Services is a 4-bed residential treatment facility located in Oakville, Ontario. We provide services for children and youth who have been diagnosed with Autism Spectrum Disorder (ASD).

We are currently looking for Full-time Case Manager. As a Case Manager you would be responsible for the following:

- Actively participating and initiating integration into community activities
- Implementing assigned individual treatment and behaviour support plans
- Provide emotional and physical support to individuals who may require assistance with daily life skills, anger management, social skills and other areas of need
- Administer medications
- Complete daily logs, incident reports, medication charts and other legal documents
- Comply with all policies and procedures as established by the agency and the Ministry of Children, Community and Social Services

Additional responsibilities include but are not limited to:

- Driving clients to and from appointments and/or community outings
- Attending appointments with clients
- Helping clients with daily hygiene routines
- Completing household chores
- Adhering to Health and Safety standards
- Interacting professionally and effectively with co-workers, parents and other professionals

Qualification Requirements
- Child & Youth Worker/Social Service Worker/Developmental Service Worker or a diploma in a related field combined with a minimum of 12 months working experience
- Experience in Applied Behaviour Analysis (ABA) is an asset, but willing to train the right person
- Understanding and Managing Aggressive Behaviours (UMAB) ) an asset, but not required, as training will be provided
- Provide clearance under the Criminal Reference Check program for Vulnerable Persons
- Pre-employment medical certificate with updated immunizations
- Current First Aid and CPR certificates
- Ability to work shift work
- Valid Ontario G class driver's license in good standing and access to own vehicle
- Have a solid command of the English language and the ability to complete formal documentation

The Employee’s duties asFull-time Case Manager include but are not limited to the following:

- A Case manager develops, with the participation of the resident/family/support network/outside involved agencies, a goal-oriented Plan of Care.
- A Case Manager coordinates, attends and chairs case conferences/plan of care meetings with the family and/or other agencies to support the child/family
- A Case Manager maintains contact with their case client’s parents, support network including Doctors, Therapists and other involved outside agencies.

**Salary**: $22.45-$25.45 per hour
