This position is ideal for a regulated health professional who brings clinical expertise, strong organizational skills, and a commitment to improving patient transitions across care settings. Reporting to the immediate supervisor, the Patient Flow Coordinator is responsible for ensuring the delivery of high-quality care and service while maintaining compliance with internal and external standards of practice.This role requires a proactive and collaborative approach to managing patient flow. The successful candidate will be expected to assess patients, coordinate admissions and discharges, and identify opportunities for process improvement.
