This position is ideal for a regulated health professional who brings clinical expertise, strong organizational skills, and a commitment to improving patient transitions across care settings. Reporting to the immediate supervisor, the Patient Flow Coordinator is responsible for ensuring the delivery of high-quality care and service while maintaining compliance with internal and external standards of practice.This role requires a proactive and collaborative approach to managing patient flow. The successful candidate will be expected to assess patients, coordinate admissions and discharges, and identify opportunities for process improvement. The Patient Flow Coordinator works closely with interdisciplinary teams to ensure patients receive timely, appropriate care in the most suitable setting.What You’ll Be DoingManages admissions, discharges, and transfers to ensure seamless transitions and optimal bed utilizationBuilds strong relationships with hospital partners and internal care teams to support timely, patient-centered careApplies critical thinking and creativity to remove barriers to flow, address risks, and escalate concerns when necessaryProvides support to staff and patients through coaching, rounding, and collaborative problem solvingContributes to continuous improvement initiatives and helps shape a responsive and flexible care systemJob QualificationWhat You BringHolds a degree or diploma in Nursing, Social Work, or Social Service Work or an equivalent combination of education and experienceHas five to ten years of clinical experience in healthcare settingsDemonstrates strong leadership, facilitation, and decision-making abilitiesMaintains a calm and collaborative approach in high-pressure situationsPossesses a valid drivers license and current CPR certificationWhy Join Us?Comprehensive Health & Dental CoverageGroup Retirement Savings PlanCompetitive CompensationSupportive, Collaborative Team EnvironmentBayshore is committed to a high quality and safe environment for both our staff and clients/patients.
