LOCATION AND ABOUT the OHT Northumberland and NHH The Ontario Health Team of Northumberland (OHT-N) is a collective of health and social service providers, patients, and caregivers working together, to provide coordinated services and supports to meet the health needs of patients and caregivers seeking care in Northumberland. The Ontario Health Team of Northumberland (OHT-N) is one of 58 Ontario Health Teams operating in the Province of Ontario. Introduced in 2019, the goal of OHTs across the province is to build a connected and sustainable health care system centred around the needs of patients and caregivers through an integrated care delivery model. OHTs aim to reduce the complexities of our current health care system in Ontario to deliver a more coordinated continuum of care for patients across multiple care settings, including, but not limited to, primary care, acute care, home care, mental health and addiction services, palliative care, and emergency services. In bringing together patients, communities, providers and system leaders as one team, regardless of organizational affiliation and/or physical location, OHTs aim to deliver health care across a specific geographical area, minimizing duplication and maximizing access. To learn more about Ontario Health and Ontario Health Teams, visit: ROLE OVERVIEW Building on initial planning for the OHT- Northumberland (OHT-N) initiatives that are already underway to improve integrated care, and patient/caregiver experiences and outcomes, the OHT-N is seeking to hire an Executive Lead. The Executive Lead will report to and be accountable to the Collaboration Council via the Co-Chairs of the OHT-N, and work with a wide range of stakeholders, staff and consultants to meet Ministry of Health (MOH) and Ontario Health (OH) timelines and deliverables. The Executive Lead will oversee a core team that provides overall OHT-N implementation support in the areas of project management, population health management, quality improvement, communications, engagement, and administration. In addition, the Executive Lead will be operationally accountable for the budget as approved by the Collaboration Council. Roles and responsibilities: Strategic Planning: The Executive Lead in collaboration with the Collaboration Council is responsible for implementing the OHT-N's strategic plan, which aligns with the provincial health strategy (Ontario Health Transfer Payment Agreement deliverables) and addresses the unique needs of the local population. This involves working with the Collaboration Council and other stakeholders to identify key annual objectives, set targets, and establish initiatives to achieve the OHT-N's goals. Collaborative Leadership: The Executive Lead will be responsible for fostering a culture of collaboration and teamwork among various healthcare and social services organizations, providers, and community stakeholders. They will be required to facilitate effective communication, build relationships, and encourage cooperation to eventually ensure the seamless integration of services across the OHT-N. Operational Management: The Executive Lead will oversee the day-to-day operations of the OHT-N, including managing financial resources, optimizing service delivery, and completing all reporting requirements. This includes hiring and supervising an OHT-N staff team to effectively develop and monitor budgets, allocate resources, and implement quality improvement initiatives to enhance efficiency and effectiveness. The Executive Lead works with the OHT-N Collaboration Council to this end in addition to overseeing and supporting the various working groups of the OHT-N. Partnership Development and Engagement: The incumbent will actively engage with OHT-N member/partner organizations and broader community partners to establish and maintain strong relationships. This involves identifying further opportunities for collaboration aligned with strategic priorities, negotiating agreements, and fostering a shared vision for integrated care delivery. Consistent with the province’s Quintuple Aim, the Executive Lead will also ensure the engagement of provider, patient, client, and caregiver voices in all work of the OHT-N as appropriate. Performance Monitoring and Evaluation: The incumbent will be accountable to establish systems and processes to monitor and evaluate the performance of the OHT-N. This includes collecting and analyzing data, measuring outcomes, and identifying areas for improvement. Policy and Advocacy: As the representative of the OHT-N, the Executive Lead will link with regional and provincial government bodies as appropriate to advocate for policy changes and funding opportunities that support the OHT-N's objectives. Communication and Public Relations: The incumbent will be accountable for overseeing the effective communication of the OHT-N's goals, initiatives, and achievements to internal and external stakeholders. Innovation and Transformation: In collaboration with the OHT-N Co-Chairs, the Executive Lead will foster a culture of innovation within the OHT-N, encouraging the exploration of new models of care delivery, technologies, and best practices. Professional Development and Team Building: The Executive Lead will support the professional growth and development of OHT-N staff members by providing mentorship, guidance, and resources. They will promote a positive work environment, encourage teamwork, and foster a culture of inclusivity and collaboration among team members. Common Responsibilities: Works in a manner that preserves confidentiality and seeks to minimize risk Incorporates and strengthens collaborative and interdisciplinary teamwork Respects and values the diversity of communities and individuals. Continues to build competence in self and in staff over time Promotes awareness of and participates in OHT-N activities Contributes to the work by participating in meetings and committees Participates in the efforts to enhance OHT-N’s capacity through staff development. Supports and complies with all Occupational Health and Safety policies and procedures QUALIFICATIONS/REQUIREMENTS 5 years +/ senior leadership, healthcare environment. Strong understanding of, and existing relationships within Northumberland County an asset. Masters level training: business administration, health administration or equivalent. Project management training and experience (PMP certification an asset). Demonstrated change management of large, complex projects including some or all of: strategic planning, digital health, quality and care delivery. Knowledge of PHIPA, data sharing agreements and privacy frameworks. Demonstrated ability to develop effective reports/presentations for various audiences including clinicians, patients etc. Demonstrated leadership, relationship building, motivational change management, innovation, and success in vertical integration across systems. Measurement, evaluation, and quality improvement experience. Experience both developing and managing budgets and reporting of financial and other metrics. Strong understanding of population health and health equity. Understanding of various health sectors, provincial healthcare policy and funding Excellent interpersonal and written communication skills, tact, and diplomacy. ASSETS Ability to work flexible hours based on program needs Valid Driver’s license Proof of COVID-19 vaccination Canadian criminal record check and a vulnerable sector check that are satisfactory to OHT-N Strong leadership and interpersonal skills, an ability to interact tactfully, professionally, and sensitively with a variety of partners An understanding of, and experience with the principles of Experience-based Co-Design Excellent communication skills, both written and oral Excellent organizational skills and ability to work independently or in a group as a team member Commits to meeting the privacy obligations and requirements of the OHT-N and its partners and protects the privacy/confidentiality of patient information in accordance with the Personal Health Information Protection Act (PHIPA) EMPLOYMENT REQUIREMENTS Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment. Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment.
