Join North York General to accelerate your career Position Summary The Breast Diagnostic Centre / Prostate Center Care Coordinator plays a pivotal role in supporting patients being referred to the prostate cancer. This position ensures seamless coordination across multidisciplinary teams, facilitates timely access to services, and provides education and emotional support to patients and families. The coordinator contributes to quality improvement initiatives and supports the delivery of patient-centered care. On a practical level, you will Serve as the primary point of contact for Breast Diagnostic Centre / Prostate center patients.Provide emotional support and connect patients with psychosocial services.Coordinate and prioritize the day-to-day functioning of the Clinic, to maximize the efficiency and effectiveness of the interdisciplinary team, resulting in optimal patient careSupport the Unit Secretary in scheduling diagnostic tests and consultations.Maintain accurate records in the hospital’s EMR system.Ensure timely communication of care plans to patients and providers.Educate patients and families about breast/ prostate cancer and treatment options.Develop and distribute educational materials.Support informed decision-making and consultation preparation.Collaborate on care pathway development and process improvements.Contribute to data collection for audits and research Qualifications Regulated Health Professional, in good standing with their corresponding regulatory authority, Registered with the College of Nurses of Ontario, preferred Minimum 3-5 years of experience in Ambulatory care, with oncology (breast) or urology experience preferred.Strong knowledge of prostate cancer care and hospital systems.Excellent communication, organizational, and interpersonal skills.Proficiency in EMR systems and care coordination tools What We Offer Working at NYGH means working with a dynamic team of fellow healthcare providers, staff, and volunteers in one of Canada’s leading hospitals.
