What we are looking for - Bachelors in social work.- Member in good standing with a professional college in Ontario.- Minimum of three (3) years of experience in mental health and addictions/gambling and concurrent disorders.- Be able to plan and coordinate care; - Be able to complete the liaison and service transition;- Have a thorough knowledge of the social context in a defined area; - Have a thorough knowledge of the functions and roles of the different care providers;- Have a good understanding of the limitations of the system's services;- Be able to build a quality relationship with the patient, family caregivers and caregivers; - Be able to empower patients- Ability to work in a multidisciplinary environment and inter-agencies- Possess a working knowledge of both official languages.
