Job Title: Visiting Nurse

We are seeking a skilled and compassionate Visiting Nurse to join our team at Closing the Gap Healthcare. As a Visiting Nurse, you will provide high-quality care to our clients in the comfort of their own homes.

About the Role:
  • Provide wound care management and treatment to clients with various wound types
  • Administer IV antibiotics and perform IV cannulation, chemo disconnects, CVAD care, and PORT-A-CATH care access/de-access
  • Work collaboratively with clients, families, and healthcare teams to develop and implement individualized care plans
  • Conduct regular assessments and evaluations to ensure optimal client outcomes
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
