About the Role
CarePartners is seeking a skilled Visiting Nurse to join our team in the Parry Sound, ON area. As a Visiting Nurse, you will have the opportunity to work in a supportive and collaborative environment, providing exceptional nursing services to patients in their homes.
Key Responsibilities
- Develop and maintain effective care plans for patients receiving community health services
- Provide one-on-one nursing services to patients in their homes, including in-person and virtual visits
- Communicate and liaise with patients, family members, and other healthcare professionals to ensure seamless care
- Perform other clinical duties as required

Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- Strong desire and commitment to making a difference in the lives of our patients
- Exceptional interpersonal skills
- Valid driver's license and vehicle
- Excellent English verbal and written communication skills
- CPR Certification
- Current immunization record, including 2-step TB test
- Vulnerable sector check and clear background check
