About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Health Care Social Worker to join our team. As a member of our interdisciplinary healthcare team, you will provide collaborative, holistic care on the primary to quaternary care continuum.

About the Role

  • Provide social work services to patients and families, focusing on psychosocial, emotional, and spiritual well-being.
  • Assess and address the social determinants of health, using a client-centered approach.
  • Develop and implement individualized care plans, working collaboratively with healthcare teams.
  • Provide education and support to patients and families, promoting healthy behaviors and coping strategies.
  • Collaborate with community partners to ensure seamless care transitions and access to local resources.

About You

  • You have a Bachelor's degree in Social Work from a recognized university.
  • You are currently registered with the Nova Scotia College of Social Workers.
  • You have at least 2 years of post-BSW clinical experience, preferably in a community-based setting.
  • You possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • You are proficient in using evidence-based treatment approaches, including Motivational Interviewing and behavior change counseling.
  • You have experience with group facilitation and are able to complete biopsychosocial assessments.
  • You are able to demonstrate effective time management and disciplined self-organization.
  • You have a valid Nova Scotia Driver's license and access to a reliable vehicle.
