Community Health Nurse Specialist

We are seeking a skilled Community Health Nurse Specialist to join our team at WRHA Community Health Services. This is an exciting opportunity for a motivated and compassionate nurse to make a meaningful difference in the lives of our clients.
Job DescriptionThe Community Health Nurse Specialist will provide high-quality nursing care to clients in various community settings, including their homes, schools, and workplaces. The successful candidate will work collaboratively with other healthcare professionals to deliver comprehensive care that addresses the unique needs of each client.
About the Role- Provide independent nursing services, including administration of prescribed treatments and facilitating coordination of client care and discharge.
- Work with clients and their families to develop personalized care plans that promote optimal health outcomes.
- Participate in multidisciplinary teams to coordinate care and address complex health issues.
- Conduct thorough assessments of clients' physical and emotional needs, identifying opportunities for intervention and support.
To be successful in this role, you will need:
- A graduate degree in nursing from an accredited institution.
- Current registration with the College of Registered Nurses of Manitoba (CRNM) and active participation in professional associations.
- Demonstrated expertise in community health nursing, including wound care and chronic disease management.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- A valid Manitoba Class 5 driver's license and reliable transportation.
While not required, the following qualifications would be considered assets:
- CNA Community Health Nurses Certification or relevant applicable certification.
- Knowledge of Indigenous customs, traditions, and values, as well as competence in an Indigenous language.
- Experience working in home care or community health settings.
This is a full-time position that requires moderate to heavy physical effort, as well as the ability to adapt quickly to changing situations and priorities. You will be expected to work independently and as part of a team, using your critical thinking and problem-solving skills to navigate complex health issues.
What We OfferAs a Community Health Nurse Specialist with WRHA Community Health Services, you will have access to ongoing education and training opportunities, as well as a supportive and collaborative work environment. We offer a competitive salary range of $83,000 - $108,000 per year, depending on experience and qualifications.
