Job Title: Community Health Nurse

At WRHA Community Health Services, we are seeking a skilled and compassionate Community Health Nurse to join our team. As a Community Health Nurse, you will provide high-quality nursing care to clients in the community, including those with complex and unpredictable health needs.

Key Responsibilities:
  • Provide independent nursing services to clients in the community, including administration of prescribed treatments and leadership roles within the healthcare team.
  • Facilitate coordination of client care and discharge, ensuring seamless transitions and optimal outcomes.
  • Work collaboratively with other healthcare professionals to provide comprehensive care and support to clients and their families.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each client.
  • Conduct thorough assessments, identify areas of concern, and develop strategies to address them.
  • Provide education and support to clients and their families, empowering them to manage their health and well-being.
  • Stay up-to-date with the latest research, best practices, and guidelines in community health nursing, ensuring that care is evidence-based and effective.
Requirements:
  • Graduate of an approved Registered Nursing education program.
  • Current registration with the College of Registered Nurses of Manitoba (CRNM).
  • Valid Manitoba Class 5 driver's license.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong critical thinking and problem-solving skills, with the ability to prioritize care and manage multiple responsibilities.
  • Ability to adapt quickly to changing situations and priorities, with a flexible and patient approach.
  • Knowledge of community health nursing standards, policies, and procedures, as well as relevant legislation and regulations.
Preferred Qualifications:
  • CNA Community Health Nurses Certification or relevant applicable certification.
  • Additional wound care and chronic disease management education.
  • Ability to speak an Indigenous language and/or knowledge of Indigenous customs, traditions, and values.
