Nurse Practitioner

We are seeking a dedicated and compassionate Registered Nurse to join our team in Primary Care at Northern Health Authority.
About the Role:The Primary Care Nurse will provide high-quality patient care, working collaboratively with the interprofessional team to deliver comprehensive care to individuals, groups, and communities. This role requires strong clinical skills, excellent communication, and a passion for delivering person-centered care.
Key Responsibilities:- Provide direct clinical care and services to patients, families, and communities
- Develop and implement care plans in collaboration with the interprofessional team
- Conduct initial and ongoing client assessments, using clinical and diagnostic reasoning to inform care
- Communicate effectively with patients, families, and healthcare teams using verbal, written, and computer communication means
- Participate in quality improvement initiatives and contribute to the development of policies and procedures
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
- Current valid BC Driver's License
