We are seeking a dedicated and compassionate Registered Nurse to join our team in Primary Care at Northern Health Authority.
About the Role:The Primary Care Nurse will provide high-quality patient care, working collaboratively with the interprofessional team to deliver comprehensive care to individuals, groups, and communities. This role requires strong clinical skills, excellent communication, and a passion for delivering person-centered care.
Key Responsibilities:- Provide direct clinical care and services to patients, including assessment, diagnosis, and treatment.
- Develop and implement individualized care plans, working collaboratively with the interprofessional team.
- Conduct health education and promotion activities to support patients and their families.
- Collaborate with community partners to provide comprehensive care and support services.
- Registration with the BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid BC Driver's License.
