Job Summary

We are seeking a skilled Community Health Worker II to join our team at Home Support Services in Burnaby, B.C. As a key member of our team, you will provide essential support to clients in their daily lives, promoting maximum independence and well-being.

Responsibilities
  1. Assist clients with activities of daily living, including personal care, housekeeping, and home management tasks.
  2. Develop and maintain care plans in collaboration with clients and healthcare professionals.
  3. Provide education and support to clients and their families on personal care, housekeeping, and home management skills.
  4. Work collaboratively with healthcare professionals to ensure seamless care delivery.
  5. Maintain accurate records and documentation of client care and services provided.
Requirements
  • Grade 12 diploma or equivalent.
  • Home Support/Resident Care Aide certificate or equivalent.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
