We are seeking a skilled Community Health Worker II to join our team at Home Support Services in Burnaby, B.C. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our clients.
Key Responsibilities- Assist clients with daily living activities to promote independence and self-sufficiency.
- Provide personal care, housekeeping, and home management services as per established care plans.
- Work collaboratively with professional staff to ensure safe and supportive care environments.
- Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery.
- Participate in quality improvement activities to maintain current knowledge and compliance with Home Health and Home Support guidelines.
- Grade 12, plus a Home Support/Resident Care Aide certificate, or an equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
