Job Summary

We are seeking a skilled Community Health Worker II to join our team at Home Support Services in Burnaby, B.C. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our clients.

Key Responsibilities
  1. Assist clients with daily living activities to promote independence and self-sufficiency.
  2. Provide personal care, housekeeping, and home management services as per established care plans.
  3. Work collaboratively with professional staff to ensure safe and supportive care environments.
  4. Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery.
  5. Participate in quality improvement activities to maintain current knowledge and compliance with Home Health and Home Support guidelines.
Requirements
  • Grade 12, plus a Home Support/Resident Care Aide certificate, or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
