About the Role

This is a part-time opportunity for a Community Health Worker II with Home Support Services located in Burnaby, B.C.

Key Responsibilities
  • Assist clients with daily living activities to promote independence and self-sufficiency.
  • Provide personal care, housekeeping, and home management services as per the care plan.
  • Work collaboratively with professional staff to ensure safe and supportive care for clients.
  • Document and maintain accurate records of client care and services provided.
  • Participate in quality improvement activities and maintain current knowledge of Home Health and Home Support guidelines, policies, and procedures.
Requirements
  • Grade 12, plus a Home Support/Resident Care Aide certificate, or an equivalent combination of education, training, and experience.
  • Valid BC Driver's License and access to a personal vehicle for business-related purposes.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Physical ability to carry out the duties of the position.
