Your Opportunity:

We are seeking a skilled Licensed Practical Nurse to join our team at Alberta Health Services. As a key member of our interprofessional healthcare team, you will be responsible for providing safe, quality patient-centered care while reflecting the shared vision and values of our organization.

Key Responsibilities:

  • Develop and implement individualized care plans to meet the unique needs of clients
  • Collaborate with clients, families, and healthcare teams to provide comprehensive care
  • Conduct thorough nursing assessments and provide direct care in clients' homes or community settings
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care transitions
  • Stay up-to-date with the latest policies, procedures, and best practices in home care

Required Qualifications:

  • Completion of an accredited practical nurse education program
  • Active or eligible registration and practice permit with the College of Licensed Practical Nurses of Alberta
  • Current Basic Cardiac Life Support - Health Care Provider certification

Preferred Qualifications:

  • Two years of recent home care or acute care experience
  • Advanced wound care knowledge and experience
  • Proficiency in computer use and ability to work independently
