Your Opportunity:
At Alberta Health Services, we are seeking a skilled Registered Nurse to join our Home Care team. As a Registered Nurse, you will provide high-quality patient care in the comfort of clients' homes, utilizing a strengths-based Client Centered Case Management practice framework. You will work collaboratively with clients and their families to identify needs, set achievable health and self-care goals, and provide direct nursing care. Your expertise will be essential in ensuring services are provided and modified on an ongoing basis, based on clients' responses to interventions.
Description:
As a Registered Nurse, you will be responsible for providing a wide range of nursing services to patients, families, and communities. You will utilize nursing processes, critical thinking, problem-solving, and decision-making to ensure safe and quality patient care. Your role will involve teaching, counseling, and advocating on behalf of patients and their families, while reflecting the shared vision and values of Alberta Health Services.
Key Responsibilities:
- Provide nursing assessments, case coordination, and direct care to clients in their homes or alternate care settings.
- Collaborate with clients and their families to develop and implement care plans.
- Utilize critical thinking and problem-solving skills to ensure safe and quality patient care.
- Teach, counsel, and advocate on behalf of patients and their families.
Requirements:
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
- Two years of recent home care or acute care experience in the last five years.
Preferred Qualifications:
- Experience working with acute clients and/or long-term clients with a focus on chronic disease management.
- Experience in providing case management services.
- Experience in Home Parenteral Therapy (HPT), wound, skin, and ostomy care, palliative support, and client teaching.
