Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Extendicare (Canada) Inc. as a Patient Care Specialist. In this role, you will provide high-quality nursing services to patients in a nursing home/home for the aged setting.

Responsibilities
  • Provide safety and health education to individuals and their families, promoting healthy lifestyles and disease prevention.
  • Assess patient needs and develop care plans to ensure optimal health outcomes, collaborating with the healthcare team to implement and evaluate interventions.
  • Monitor patients' progress, evaluating the effectiveness of nursing interventions and making adjustments as needed.
  • Administer medications and treatments as prescribed, maintaining accurate records and reporting any adverse reactions.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care.
Requirements
  • Completion of the Canadian Practical Nurse Registration Examination.
  • Basic Cardiac Life Support (BCLS) Certificate.
  • CPR Certificate.
  • Criminal record check.
  • Immunization records.
  • Tuberculosis test.
  • Vulnerable sector check.
