Healthcare Social Worker

Nova Scotia Health is seeking a skilled Health Care Social Worker to join our team in the Western Zone, Yarmouth Regional Hospital. As a member of our interprofessional team, you will play a vital role in delivering Primary Health Care and chronic disease management services, with a focus on chronic pain.
The successful candidate will possess a Master's degree in social work from a recognized university and current registration with the Nova Scotia College of Social Workers. They will have a minimum of 2 years of post-MSW clinical experience and a proven ability to use a client-/family-centered approach. Experience working in Primary Care and/or community-based settings is preferred.
The ideal candidate will be able to demonstrate effective time management and disciplined self-organization, as well as the ability to work collaboratively with an interdisciplinary team and community partners. They will also possess excellent communication skills and the ability to assist patients and families with navigation of local community resources and agencies.
This is a permanent full-time position, with 75 hours bi-weekly. The successful candidate will be required to travel to various sites and may be required to work evenings and weekends.
The compensation for this position is $37.31 - $49.04 hourly, with the possibility of eligibility for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
