Job Title: Social Services Coordinator

Job Summary:

We are seeking a skilled Social Services Coordinator to join our team at Centre intégré de santé et de services sociaux de la Côte-Nord. As a Social Services Coordinator, you will be responsible for providing support and guidance to clients in need, working closely with healthcare providers and other social services agencies.

Key Responsibilities:

  • Prepare reports and case histories to document client progress and needs
  • Implement life skills workshops to promote client independence and well-being
  • Assess client needs and eligibility for specific services, working closely with healthcare providers and other social services agencies
  • Conduct follow-up assessments to monitor client progress and adjust support services as needed
  • Assist in evaluating the effectiveness of treatment programs and making recommendations for improvement

Requirements:

  • Bachelor's degree in a related field, such as social work or psychology
  • 1-2 years of experience in a related field, such as social services or healthcare
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, healthcare providers, and other social services agencies
  • Ability to work under pressure and prioritize tasks effectively
