We are seeking a skilled Patient Services Partner to join our team at Southlake Regional Health Centre. As a Patient Services Partner, you will play a vital role in providing patient care to a specific group of patients under the supervision of Regulated Health Care Professionals.
Key Responsibilities- Provide physical assistance to patients as required, including assisting with lifts and transfers, ambulation, toileting, bathing, and personal hygiene functions under the direction of a Regulated Health Care Professional.
- Assist with tray set-up and feeding of patients as directed by a Regulated Health Care Professional, including opening packages, making patients comfortable, and documenting food consumption.
- Monitor at-risk patients by direct observation in or outside the patient room.
- Document patient care activities in the Health Care Record.
- High school diploma required.
- Successful completion of a Personal Support Worker/Developmental Service Worker program through a National Association of Community Career Colleges approved program, or successful completion of a minimum of the 1st year of a Nursing program.
- Recent patient care experience preferred.
- Demonstrates understanding of infection prevention and control practices.
- Demonstrates conflict resolution skills.
- Demonstrated problem assessment skills.
- Demonstrated commitment to customer service and teamwork.
- Demonstrated individual leadership skills.
- Commitment to providing an exceptional experience for patients and families as well as other members of the healthcare team within a challenging and exciting healthcare environment.
