At Southlake Regional Health Centre, we are seeking a highly skilled and compassionate Geriatric Care Specialist to join our team of healthcare professionals. The ideal candidate will have a strong background in geriatric care and a passion for delivering high-quality patient-centered care.

Job Summary

The Geriatric Care Specialist will work collaboratively with our inter-professional team to provide comprehensive geriatric care to elderly patients in their homes and communities. This will involve conducting thorough assessments, developing personalized care plans, and providing education and support to patients and their families.

Key Responsibilities

  • Conduct comprehensive geriatric assessments in patients' homes and communities
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Provide education and support to patients and their families on geriatric care and disease management
  • Collaborate with our inter-professional team to ensure seamless care coordination
  • Participate in weekly team rounds to discuss patient progress and care plans

Qualifications

  • Bachelor of Science in Nursing (BScN) or equivalent
  • Registered and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse
  • Current BCLS certification
  • Three to five years of experience in geriatric care, preferably in an acute medicine or community setting
  • Strong knowledge of community-based services available to seniors
  • Valid driver's license and access to own vehicle for community visits
