Addiction Services Nurse Practitioner

Role Summary:
The Addiction Services Nurse Practitioner provides comprehensive patient care to individuals with mental health and addiction needs. This role involves working directly with patients to assess their needs and develop personalized care plans.
Key Responsibilities:
• Assess patients' physical and emotional health needs
• Develop and implement individualized care plans
• Collaborate with healthcare teams to ensure comprehensive care
• Provide education and support to patients and their families
Qualifications:
• Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
• Minimum of two to three years of community health care and/or primary care experience
• Master's Degree (Preferred)
• Experience in addiction medicine and harm reduction (Strongly recommended)
