About the Role


Vancouver Coastal Health is seeking a dedicated and compassionate Patient Care Specialist to join our In-Patient Psychiatry team at Sechelt Hospital.



Job Summary


The successful candidate will provide comprehensive mental health care services to patients within our designated Program(s). This includes conducting thorough assessments, developing and implementing personalized care plans, and providing crisis interventions as needed.



Main Responsibilities



  • Provide direct patient care services, including assessment, treatment, and support for individuals with mental health and substance use concerns.
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans that address individual patient needs.
  • Conduct thorough assessments of patients' physical and emotional well-being, identifying areas for improvement and developing targeted interventions.
  • Develop and maintain effective relationships with patients, families, and healthcare providers to ensure seamless care coordination.
  • Act as a liaison between healthcare services and community organizations to facilitate access to resources and support.
  • Stay up-to-date on best practices in mental health care and incorporate this knowledge into daily practice.
  • Document patient interactions and progress accurately and thoroughly in accordance with organizational standards.

Qualifications


To be considered for this role, you must possess:



  • A current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three years' recent related experience providing treatment services to individuals with mental health and substance use concerns.
  • Valid BC Driver's License and reliable transportation.
  • Demonstrated ability to communicate effectively, both verbally and in writing.
  • Able to prioritize tasks, manage multiple responsibilities, and work collaboratively as part of a multidisciplinary team.
