Job Summary:

The Assessor Coordinator Degree Specialist will provide case management services, including assessment, planning, monitoring, and evaluation. This role is responsible for single entry access and ongoing coordination for community-based services within the Saskatchewan Health Authority.

Key Responsibilities:

- Provide assessment and planning services for clients
- Coordinate community-based services
- Monitor and evaluate client progress

Requirements:

- Bachelor of Social Work (BSW) or equivalent
- Knowledge of SHA community resources and programming
- Assessment knowledge

Preferred Qualifications:

- Registered/eligible for registration with the appropriate licensing body
- Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
