Primary Care Nurse

We are seeking a highly skilled Registered Nurse to join our Primary Care team in Smithers. As a Primary Care Nurse, you will provide compassionate patient care and work collaboratively with our interprofessional team to deliver high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities
- Develop and implement care plans in collaboration with the interprofessional team
- Conduct assessments and provide education to patients and families
- Work effectively in a team environment to promote continuity of care
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings
- Current valid driver's license
