Compassionate Primary Care Nurse

Are you passionate about nursing? We are looking for a committed Registered Nurse who is dedicated to providing compassionate care to patients in the realm of Primary Care.
In alignment with Northern Health's vision of an integrated system of services, the Primary Care Nurse (PCN) operates within the standards of professional practice and ethical guidelines set forth by the BC College of Nurses and Midwives (BCCNM).
The PCN is an integral member of the interprofessional team, utilizing best practices to deliver competent, safe, and ethical care to individuals and families in various settings, including clinics, homes, schools, and community environments.
The role involves delivering care that encompasses clinical follow-ups based on the individual’s Primary Care Plan and personal health goals, contributing to collaboratively developed care plans with the interprofessional team.
Work Schedule: Monday to Friday, Day shifts - 08:30 to 16:30
Primary and Community Care Approach

- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years of recent, relevant clinical nursing experience in primary care and/or chronic disease management, including experience in patient education.
- Current valid Driver's License.
- Assessment and Intervention - Proven ability to conduct initial and ongoing client assessments and provide nursing care through appropriate interventions.
- Communication - Strong ability to communicate effectively with clients, families, the public, and interdisciplinary team members.
- Critical Thinking - Ability to analyze and evaluate data from multiple sources to solve problems effectively.
- Person and Family-Centered Practice - Commitment to promoting care that respects the individual and their family within their community context.
- Teaching and Mentoring - Knowledge of adult learning principles to educate patients and families effectively.
- Management - Efficiently manages time and resources to support service delivery.
- Leadership - Encourages staff morale and collaboration while promoting professional development.
- Knowledge Integration - Utilizes evidence-based practices and integrates knowledge from various disciplines into nursing practice.
- Population Health - Develops a core understanding of population health principles and values.
- Physical capability to perform the duties of the position.
