We are seeking a highly motivated and dedicated Registered Nurse to join our team in Primary Care at Northern Health Authority. As a Primary Care Nurse, you will play a vital role in delivering compassionate patient care and working collaboratively with our interprofessional team to provide high-quality services to our patients.
Key Responsibilities- Provide direct clinical care and services to individuals, groups, and communities in Primary Care settings.
- Develop and implement care plans in collaboration with the interprofessional team, taking into account the patient's primary care plan and personal goals.
- Conduct clinical follow-ups and provide education to patients, families, and the general public on health-related topics.
- Work collaboratively with the interprofessional team to redesign and improve our primary and community care system.
- Participate in shift rotation and work hours as required.
- Registration with BC College of Nurses and Midwives as a practicing RN or RPN registrant.
- Three years recent, related clinical nursing experience in primary care nursing and/or chronic disease management settings.
- Current valid driver's license.
- Experience developing and providing formal education to patients, families, and the general public.
- Knowledge of adult learning principles and facilitation skills to effectively educate people and others about topics essential to health care and well-being.

- Four weeks vacation with one year of continuous service.
