Licensed Practical Nurse

As a Licensed Practical Nurse (LPN) at Crosstown Clinic, you will play a vital role in providing high-quality patient care to an assigned group of patients. Reporting to the Patient Care Manager or Resident Care Manager, you will work collaboratively with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities- Conduct thorough head-to-toe assessments to identify patient needs and develop care plans
- Provide safe and respectful nursing care to patients, including administering medications and monitoring their effects
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
- Participate in care conferences and contribute to the development of individualized care plans
- Assist with the orientation of new staff and students, demonstrating work procedures and sharing clinical knowledge
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
- Current basic cardiac life support certification
