We are seeking a skilled Registered Practical Nurse to join our team as a Community Care Specialist in Oakville. As a key member of our nursing team, you will provide high-quality care to patients in their homes, promoting independence and comfort.
About the Role
This is a part-time position offering every other weekend off, with attractive scheduling and opportunities for career advancement. You will work closely with our RNs to provide comprehensive care to patients of all ages, managing complex needs and building long-term relationships.
Key Responsibilities
- Participate in scheduled community visits to assess patient needs and develop personalized care plans
- Provide safe and effective nursing care, utilizing assessment tools and techniques to inform your practice
- Maintain accurate and up-to-date patient records, reflecting encounters and care plans
- Engage patients using a collaborative approach, fostering rapport and trust
Requirements
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care (an asset)
- Reliability, flexibility, and willingness to travel to patient residences within designated areas
