Job Summary

We are seeking a skilled and compassionate Assertive Community Treatment Team Member to join our team at Island Health. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities
  • Assessment and Treatment Planning: Conduct comprehensive assessments of clients' mental health needs and develop individualized treatment plans to address their unique challenges.
  • Case Management: Provide ongoing case management services to clients, including coordination of care with other healthcare providers and community resources.
  • Community Outreach: Establish and maintain relationships with community partners and stakeholders to ensure seamless care coordination and access to resources.
  • Team Collaboration: Work collaboratively with a multidisciplinary team of healthcare professionals to provide comprehensive care and support to clients.
  • Professional Development: Stay up-to-date with best practices in mental health care and participate in ongoing professional development opportunities to enhance your skills and knowledge.
Requirements
  • Education and Experience: Graduation from a recognized nursing program and a minimum of one year's recent psychiatric nursing experience with adults with a variety of mental disorders.
  • Registration and Certification: Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant, and current CPR certification.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, and strong critical thinking and problem-solving skills.
Working Conditions
  • Travel Requirements: Valid Class 5 BC driver's license and use of a personal vehicle available to travel between work sites.
  • Physical Demands: Ability to perform the duties of the job, including lifting and transferring clients as required.
