Hamilton Health Sciences is seeking a skilled Registered Nurse to join our team as a Nurse Leader in the Seniors, Emergency, and Community Medicine Program. As a valued member of our healthcare team, you will provide leadership in the coordination of nursing care for patients with multisystem medical conditions.
Key Responsibilities- Coordinate and provide nursing care for patients with complex medical conditions, including diabetes, dementia, stroke, and acute coronary syndromes.
- Collaborate with the interdisciplinary healthcare team and community agencies to provide comprehensive care for patients and their families.
- Develop and implement evidence-based clinical practices to improve patient outcomes and healthcare delivery.
- Provide leadership and guidance to nursing staff to ensure high-quality patient care.
- Participate in quality improvement initiatives to enhance patient safety and satisfaction.
- Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse.
- Current Basic Cardiac Life Support (BCLS) certification.
- Previous related experience in acute medicine or geriatric care.
- Demonstrated leadership, critical thinking, decision-making, communication, organizational, problem identification, and problem-solving skills.
- Knowledge of current nursing assessment and management of adult and geriatric medicine patients, including rehabilitative approaches to patient care.
- Demonstrated ability to work independently while collaborating with an interdisciplinary healthcare team.
