Psychiatric Liaison Nurse, Community Mental Health Specialist

The Psychiatric Liaison Nurse is a vital member of our interprofessional care team, providing person-centered and recovery-oriented care to psychiatric patients in the Emergency Department. This role involves performing intake assessments, offering crisis intervention, and providing input into the care of patients awaiting transfer for further assessment or inpatient admission.
Key Responsibilities
- Screen psychiatric referrals from internal and external sources, gather background information, and determine service priorities.
- Perform psychosocial assessments, collecting collateral information on patients' support groups, social environments, occupations, education, housing, finance, health service access, and legal problems.
- Develop care plans with final dispositions authorized by Emergency Room Physicians or consulting Psychiatrists, providing general and crisis counseling to patients and their families.
- Communicate and consult with Coordinators and other mental health professionals on patient care issues, resources, and discharge planning.
- Provide patient advocacy services, document assessments, care plans, and interventions according to practice standards and Fraser Health procedures.
Requirements
We are seeking a skilled Registered Nurse or Registered Psychiatric Nurse with one year of recent experience in acute or community mental health settings. The ideal candidate will possess strong communication and interpersonal skills, knowledge of mental illness and substance use treatment, and the ability to promote person-centered care.
