Vancouver Coastal Health is seeking a skilled Geriatric Care Physician to join our team at Vancouver General Hospital.
Key Responsibilities:- Develop and implement safe discharge plans in collaboration with acute care services, community primary care physicians, and community/Home Health services.
- Provide patient and family-centered care.
- Support the Emergency Department in managing complex vulnerable Geriatric patients, to avoid hospital admission when appropriate.
- Develop collaborative trusting professional relationships with interdisciplinary team members, health authority staff, and community physicians.
- Liaise with relevant community primary care providers and specialists to support safe discharge plans in the Frail Elderly.
- Communicate clearly with patients, families, and other healthcare providers.
- Initiate and engage with patients and family members in challenging Goals of Care discussions.
- Provide medical oversight for home health nurses providing care for patients in the community.
- Partner with Home and Community Health to promote team-based care to prevent unnecessary ER visits and admissions.
- Current College of Physicians of BC (CPSBC) License - Full.
- Canadian Medical Protective Association (CMPA) – Mandatory.
- Canadian College of Family Practitioners (CCFP) – Mandatory.
