Geriatric Care Opportunity

Vancouver Coastal Health is seeking a skilled EDiCare Physician to join our team in the Emergency Department at Vancouver General Hospital. As a Geriatric Care Specialist, you will play a vital role in providing high-quality care to our frail elderly patients.

Key Responsibilities:

  • Develop and implement safe discharge plans in collaboration with acute care services, community primary care physicians, and community/Home Health services.
  • Provide patient and family-centered care, focusing on the unique needs of our geriatric patients.
  • Support the Emergency Department in managing complex vulnerable Geriatric patients, avoiding hospital admission when possible.
  • Liaise with community primary care providers and specialists to support safe discharge plans.
  • Communicate clearly with patients, families, and other healthcare providers.

Qualifications:

  • Current College of Physicians of BC (CPSBC) License - Full
  • Canadian Medical Protective Association (CMPA) – Mandatory
  • Canadian College of Family Practitioners (CCFP) – Mandatory
