CarePartners, one of Ontario's largest accredited home health care providers, seeks a dedicated Physiotherapy Specialist to join its dynamic Rehabilitation Team. This full-time position is based in the community and requires regular travel to patients' homes.

Key Responsibilities
  • Conduct professional assessments of patients in their homes and develop effective care plans in collaboration with the Home and Community Care Support Services (HCCSS)
  • Work with patients to set measurable goals and outcomes and provide guidance on workplace health and safety
  • Provide one-on-one therapeutic support to clients and promote their independence in the home and community
Requirements
  • Master's Degree in Physiotherapy from a recognized university
  • Registration with the College of Physiotherapists of Ontario
  • Proven computer skills and strong assessment skills
  • Excellent verbal and written communication skills in English; French language skills are an asset
