Job Summary
The Nurse Practitioner - Renal Care Specialist will provide direct clinical care, education, research, consultation, and treatment to support the care needs of designated renal patients. This role will function as an independent practitioner and as a collaborative partner within the inter-professional team.
Key Responsibilities
- Performing and documenting medical history and physical exam findings in the Electronic Medical Record (EMR)
- Diagnosing medical conditions
- Ordering appropriate tests/procedures in consultation with physician partners
- Interpreting laboratory/diagnostic imaging and other test results
- Prescribing pharmacologic and non-pharmacologic therapies independently or in consultation with physician partners
- Consulting as needed with the Hospitalists, Specialists, Community Primary Care Practitioners, and Allied Health team members
- Utilizing diagnostic reasoning and decision-making process to formulate a list of hypotheses, determine the need for, order, and interpret diagnostic testing, and formulate a medical diagnosis within the specialty scope of practice and consistent with practice guidelines
- Initiating/orders, tailoring, and evaluating/discontinuing selected medical and nursing therapy
- Performing specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts, and established practice guidelines
- Supervising/precepting nursing students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience and evaluating their performance
Qualifications
- Current registration with the College of Nurses of Ontario is required
- BScN required
- Nurse Practitioner Certification required
- Masters degree in Nursing/Nurse Practitioner Stream is preferred
- Acute Care Nurse Practitioner Certification preferred
- Gerontology Nursing Certification is preferred
- BCLS is required
Experience
- Minimum three (3) years experience in advanced practice nursing in adult medicine/geriatrics/eldercare preferred
- Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required, experience in Acute Care preferred
- Recent experience in a leadership role within an inter-professional team preferred
- Relevant knowledge of inpatient care environment preferred

- Demonstrated program development skills and experience

- Demonstrated commitment to the concepts of Continuous Quality Improvement and Customer Service
Other
- Current registration with the College of Nurses of Ontario is required
- Current member of RNAO
- Clinical specialty organizational membership
- Current member of the Nurse Practitioner's Association of Ontario
- Demonstrates commitment to continued professional and personal growth and development through Continuing Education Programs and Self-Directed Learning Opportunities
- Must have a valid driver's license and a reliable vehicle to provide client care/services off-site
- Computer literacy working in Windows environment
