Hospice Palliative Care Nurse Practitioner

We are seeking a skilled and compassionate Hospice Palliative Care Nurse Practitioner to join our team at Home and Community Care Support Services. As a key member of our inter-professional care team, you will provide expert clinical care to individuals with complex palliative care needs in the community.
Key Responsibilities:- Provide comprehensive clinical assessments and develop shared care plans in collaboration with patients, families, and healthcare teams.
- Manage pain and other symptoms, including therapeutic interventions and pharmacology.
- Coordinate interdisciplinary plans of care and facilitate navigation of the healthcare system.
- Facilitate ethical decision-making and communicate plans of care to patients, families, and healthcare teams.
- Deliver education and professional development activities to support clinical care and quality improvement initiatives.
- Current registration with the College of Nurses of Ontario in the Extended Class Nurse Practitioner Program.
- Certification as a hospice palliative care nurse with the Canadian Nurses Association CPHCN(C).
- Minimum 2 years of experience in palliative care nursing, with previous work in the community setting an asset.
- Valid Ontario driver's license and access to a vehicle.
- Proficient in a Windows environment.
