Job Title: Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse to join our Transitional Care Centre team in West Bedford, Nova Scotia.

About the Role:

As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care, conducting health assessments, and developing individualized care plans. You will work collaboratively with our interdisciplinary team to ensure optimal health and wellness for each patient.

Key Responsibilities:
  • Conduct initial and regular health assessments to maintain patient quality of life
  • Develop and implement individualized care plans that focus on patient functional abilities and support network
  • Assess, plan, implement, and evaluate nursing care of patients
  • Ensure compliance with our Philosophy of Service and patient care plan
  • Manage patient medication in accordance with legislative requirements
  • Administer and record prescribed medications in accordance with facility policy
  • Provide consultation regarding patient issues and standards of care to the interdisciplinary team
  • Communicate with all other departmental teams to increase awareness of patient health changes
Requirements:
  • Diploma in Practical Nursing and current registration with the Nova Scotia College of Nursing
  • Comprehensive knowledge of nursing and healthcare practices, excellent communication and interpersonal skills
  • Availability to work a schedule of flexible hours and all shifts, including days, evenings, nights, weekends, and holidays
  • Advanced proficiency in the English language, with proficiency in other languages considered an asset
  • Ability to provide a CPR & Emergency First Aid Certificate, Clear Criminal Record Check, and Vulnerable Sector Screening upon hire
