Program Support Specialist

About the Role:
We are seeking a Program Support Specialist to join our team at Baycrest. This is a dynamic role that requires a strong understanding of dementia care and a passion for delivering exceptional support to individuals living with cognitive impairment.
Job Summary:
The successful candidate will be responsible for providing direct support to residents, promoting their independence and autonomy, and collaborating with the interdisciplinary team to develop and implement individualized care plans.
Main Responsibilities:
- Deliver high-quality support services to residents, including personal care, meal preparation, and activity planning
- Develop and maintain relationships with residents, families, and care providers to ensure seamless transitions and effective communication
- Collaborate with the team to create and implement daily program plans, activities, and events that promote engagement and enjoyment
- Support residents in maintaining their physical and emotional well-being through proactive identification and management of risk factors
- Provide education and training to staff on best practices in dementia care, person-centered care, and trauma-informed care
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to dementia care
Requirements:
- Diploma or degree in a relevant field (e.g., gerontology, social work, nursing)
- Current registration as a Personal Support Worker (PSW) in Ontario
- Experience working with individuals living with dementia, preferably in a residential setting
- Excellent communication, interpersonal, and problem-solving skills
- Able to work independently and collaboratively as part of a multidisciplinary team
Benefits:
- $40,000 - $50,000 per year (dependent on experience)
