We have an exciting opportunity for a Cardiac Care Program Coordinator - Patient Flow Specialist to join our team. In this role, you will play a vital part in ensuring efficient patient flow across the Regional Cardiac Care Program.
About the RoleThis is a challenging and rewarding position that requires strong leadership skills and the ability to work collaboratively with the interprofessional team. The successful candidate will be responsible for coordinating daily staffing, ensuring quality patient care delivery, and implementing evidence-based practices within the program.
The Cardiac Care Program Coordinator - Patient Flow Specialist will also be responsible for:
- Coordinating material and human resources to ensure smooth patient flow;
- Liaising with members of the interprofessional team to ensure excellence in patient care;
- Maintaining high standards of patient care through monitoring of standards of care and patient outcomes;
- Collaborating with the manager regarding human resource responsibilities, including recruitment, just-in-time teaching, and ongoing performance evaluation.
Requirements
- Certificate of registration from the College of Nurses of Ontario (CNO);
- Baccalaureate Degree in Nursing Science (preferred);
- Minimum of 2-5 years recent and relevant cardiac care experience;
- Demonstrated knowledge in clinical practice in the Cardiac Program;
- Ability to communicate and work effectively with all members of the interprofessional team;
- Strong problem-solving skills and the ability to make sound clinical decisions.
