Your Role

You will use your clinical skills to provide high-quality home health care services to our clients, working independently and as part of a team to ensure that each client receives the personalized attention and support they need. You will also be responsible for communicating effectively with clients, families, and healthcare professionals to ensure seamless care coordination and optimal outcomes.

Key Responsibilities

  • To provide comprehensive home health care services, including wound care, medication management, and mobility assistance;
  • To assess clients' needs and develop personalized care plans to meet their unique requirements;
  • To work collaboratively with other healthcare professionals to ensure that clients receive coordinated and holistic care;
  • To maintain accurate and timely records of client interactions and care provided;
  • To participate in ongoing education and professional development to enhance your clinical skills and knowledge;
  • To contribute to a positive and respectful work environment, promoting a culture of safety, quality, and patient-centered care.

What We Offer

We offer a competitive salary range of $80,000 - $110,000 per year, depending on experience, as well as a comprehensive benefits package, opportunities for career growth and development, and a supportive work environment that values diversity, equity, and inclusion. Requirements

To be successful in this role, you will require:

  • A valid driver's license and reliable vehicle;
  • A current registration with the provincial licensing body;
  • A valid First Aid certification (or willingness to obtain);
  • Excellent communication and interpersonal skills;
  • A strong understanding of home health care principles and practices;
  • The ability to work independently and as part of a team;
  • The ability to adapt to changing situations and priorities.
