We are seeking a highly skilled and compassionate Registered Practical Nurse (RPN) to join our team at Schlegel Villages. As an RPN, you will play a vital role in providing high-quality care to our residents, promoting their physical, emotional, and social well-being.
Key Responsibilities- Provide direct care to residents, including administering medications, wound care, and other treatments as needed.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Conduct regular assessments and evaluations to identify residents' needs and preferences.
- Collaborate with the Neighbourhood Physician and other healthcare professionals to ensure seamless care coordination.
- Communicate effectively with residents, families, and colleagues to ensure a positive and supportive environment.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current registration with the College of Nurses of Ontario.
- Existing Heart Save Level of CPR.
- Ability to prioritize tasks effectively and manage stress in a fast-paced environment.
- Strong communication and interpersonal skills.
- Ability to work collaboratively as part of a multidisciplinary team.
- Knowledge of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector.
- MDS credentials.
- Experience working with people living with Dementia.
- Knowledge of the palliative assessments scale.
- Comfortable supporting residents and their families during end of life care.
