We are seeking a skilled Registered Nurse (RN) or Registered Practical Nurse (RPN) to join our team as a Community Health Care Specialist in the Britt, Byng Inlet and Pickerel area. As a member of our interdisciplinary care team, you will provide exceptional nursing services to patients in their homes, promoting independence, dignity, and comfort.

Estimated Salary Range: $95,000 - $110,000 per year

Job Description
  • Develop and maintain effective care plans with respect to community health services
  • Provide exceptional nursing services to patients in their homes, both in-person and virtually
  • Communicate and liaise with patients, family members, and other healthcare professionals within the interdisciplinary care team
  • Perform other clinical duties as required
Required Skills and Qualifications
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • Strong interpersonal skills and a commitment to patient-centered care
  • A valid driver's license with reliable transportation
