Job Description:As a Home Health Care Nurse with CarePartners, your role will involve providing comprehensive nursing services to patients in their homes, schools, clinics, and workplaces. You will work within an extremely supportive environment focused on quality patient care, exercising your full scope of practice and pursuing specialization.

You will have the opportunity to:

  • Develop and maintain effective care plans with respect to community health services
  • Work with patients to provide exceptional nursing services, both in-person and virtually
  • Communicate and liaise with patients, family members, and other healthcare professionals

A valid driver's license and vehicle are required for regular travel. Excellent English verbal and written communication skills are essential for this role.

Salary:

The estimated salary for this role is $85,000 - $115,000 per year, depending on experience and qualifications.
